All personal information held by Mr J Therapy is treated as private and confidential. It is used solely by Mr J Therapy and will not be disclosed to other parties.
The client understands that the therapist may refuse to undertake treatment should this be contraindicated due to medical or other conditions that the client may present with, due to concerns regarding the health, safety and wellbeing of either the client or the massage therapist.
Therapists reserve the right to refuse or terminate treatment to any client, at her own discretion, refuse, discontinue or defer treatment. In this instance full payment is still required and no refund will be given.
Appointments can be made via email or phone. New clients receive a free 15 minute consultation which allows time to complete your health intake form; this ensures a 60 minute massage treatment without time taken completing forms. Your appointment arrival time will be agreed to when you book. We allow plenty of time between appointments to ensure that you start on time.
Mr J Therapy accepts payment in cash, by cheque, bank transfer or through PayPal. Invoices will be sent to clients who do not pay for, or who cancel, treatments and must be paid within 7 days of the issue date.
A deposit of £50 will be charged to your credit or debit card at the time of booking, in order to secure the appointment.
The deposit will go towards the cost of the treatment or consultation on the day of your appointment.
Please note 24 hours notice is required for all cancellations. Cancellations made less than 24 hours before the appointment time will be subject to a charge of 100% of the total treatment that was booked. The cancellation fee must be paid within 7 days of the cancellation.
Mr J Therapy has the right to cancel a treatment by giving 24 hours notice.
Mr J Therapy has the right to cancel a treatment without notice if the room or environment is unsuitable for the treatment to be provided, or if the therapist is taken ill.
Mr J Therapy commits to rearranging treatments to a new time and date within two weeks of the cancelled treatment where possible.
Client treatment details will not be discussed with anyone other than the client unless the client is under the age of 18 or has a care worker or guardian.
Consultations will only be discussed with the client unless the client is under the age of 18 or has a care worker or guardian. Consultation Forms and treatment notes will be kept on file, securely.
All clients must complete a consultation form prior to receiving Mr J Therapy treatments.
By signing the consultation form, clients are giving Mr J Therapy permission to hold records and data about them. Personal information will not be shared with any third parties.
Clients under the age of 18 must be accompanied by an adult. The adult must give permission for the treatment to be performed.
The adult must co-sign the client’s consultation form and must be present in the room throughout the treatment.
